Currently Vaccinating Phase All Phases for anyone 12 years and above.

Step 1:  Schedule your COVID-19 vaccination

If this is your second dose

You must bring your COVID-19 Vaccination Record Card to your second dose appointment.
If your first dose was Moderna, your second dose can be received 28 days or after.
If your first dose was Pfizer, your second dose can be received 21 days or after.
If you have received the Johnson & Johnson vaccine, you only need one dose.

Step 2:  Complete the Screening/Consent Form

Please print and fill out the form to bring with you to your appointment. If you cannot print the form, blank forms will be available at the site.

Step 3:  Arrive at the vaccination site at your scheduled time with your form and photo ID

Step 4:  Plan for onsite observation time up to 30 minutes following your vaccination
